Print and complete the appropriate application, which can be downloaded from the Home page. Make a photocopy of a single voided check on a separate piece of paper. Then fax the application and copy of the check to 636.552.0018. If you choose not to pay via EFT (Electronic Funds Transfer), you can be direct billed. There is a $4.75 charge / month for this option. This fee is waived if you pay EFT, Quarterly, Semi-Annually, or Annually. Of course the application and payment can be mailed to us if that happens to be your preference. Finally, we would be more than happy to come out and personally assist with the application process.
